When you come for your appointment please bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
  • List of medications (if any)

If your appointment is for a second opinion or a consultation for a revision joint replacement, please bring the following information if possible:

  • All operative reports for all surgeries performed on the replaced joint, including implant records (these are generally found at the hospital/surgery center your surgery was performed at).
  • Bring any laboratory data/bloodwork if you have it, including ESR, CRP, CBC, BMP, joint cultures, etc. 
  • Bring any previous imaging, including CT scans, MRIs, and bone scans if available. Please do not just bring the reports, the images will need to be reviewed. These are typically put on a disc that can be uploaded in our office.
